Instructions: Please print and fill out all fields. We welcome new members in the Leschetizky Pianist, Pianist, and Friend categories. Membership includes
receipt of the Leschetizky Association News Bulletin and participation in all
activities and concerts of the Association. To apply for membership you must be sponsored by a current member in good standing.
A check payable to The Leschetizky Association, Inc., must accompany this
